I have created the survey...and submitted my answer. But when I go into Survey Maker/Statistics for that survey...all I see are 0's for the responses. Is it possible that my survey is not working?

Your survey is not set up correctly. It's the same problem that happened with somebody else in this thread.

You have a 5 question survey currently, but what you really need to have is a 1 question survey with 5 possible answers all tied to the same question.

I'm going to put in a note to the programmer to not allow surveys with radio button answer types to be saved unless there is 2 or more answers from which to choose so nobody else can do this in the future.
