Set up multiple choice Survey. However, clicking on Radio button does not produce any results when surveys are submitted.

You added several QUESTIONS. What you needed to do is add ONE QUESTION "Which School?" and then add 11 different answers. Please check out your survey, question #12 - I did it for you. You can compare it to what you did. Then you probably want to delete your questions 1-11.
