Contact Us Form Editable? Part 1

I need help finding where on our Contact Us Page you can see the Form for people to submit. I can see it on Preview and displayed pages, but not on the Edit Site Page. On the Preview Page, I can see it's not displaying correctly ("*Your Name is to the right of my table). It also doesn't work (at least with my test submission). Displayed Page Edit Page

The code added above the form was affecting the form. I adjusted it slightly so that the form Name is treated as a separate paragraph. Jessica
