Howdy, I'm using Survey Maker to build a planning survey for our 40th Reunion in 2014. One question I would like to ask is, "Which month would you most likely be able to, or prefer, to attend our BDC 40th Reunion?"

Using the standard radio button response option, I've listed all twelve months and respondents can select only one month option as their answer. I would like enable them to provide up to three different month answers but it doesn't appear that Survey Maker will permit this. It looks like respondents are limited to one answer only. Is there any way to permit multiple responses (besides having them answer in a free form text response answer)?

Yes you can choose the Check Box option rather than Radio Button and it will allow them to check off more than one option.
